Aetna Medicare PPO, Ameritas, Anthem BCBS Prime/Complete, Cigna DPPO Total Network plans Only, GEHA, Guardian as of 12/1/25, Humana Medicare PPO, Principal, UMR, United Healthcare.

We are out of network with Medicaid, All HMO plans, Delta Dental, Metlife, Cigna DPPO Advantage Network plans, Anthem BCBS GRID Dental plans, Some Aetna plans, Dental Health Alliance (DHA), Dentegra, Sunlife, and United Concordia.
